REQUEST ASSISTANCE

We will consider any reasonable request for assistance from military personnel and their families, including active duty, National Guard, reserve, retired service members, and honorably discharged veterans. Requests will be evaluated by AFSC and approved or disapproved based on need and funds available. AFSC will periodically re-evaluate and adjust its criteria for providing assistance, according to overall needs and resources available. To apply for assistance, simply follow these steps:

  1. Call us to discuss your request. By speaking with us first, we can determine what documentation needs to be submitted with your application so that your request can be processed quickly. We can also discuss whether or not your request fits within our criteria. Call us at 970/247-9186 or 970/247-9160.

  3. Fill out the form completely and gather any pertinent documentation. If requested information is not included, it will slow down the processing of your application.
  4. Send us the completed form, along with documentation. Send it to us by mail or FAX, as follows:

  5. AFSC will review and act on your request. We will do this as quickly as we can. During the course of our review, we will verify the information on your application and may call you for further discussion of your request and to ask for additional information or documentation, if necessary. Upon completion of our review, we will contact you with our decision. If your request for financial assistance is approved, disbursements will be made directly to providers. For example, if you requested assistance to pay an energy bill, we would pay the energy company directly.
